Les prélèvements prescrits par les généralistes et réalisés dans les drives

Le parcours de dépistage Covid-19 évolue à partir de cette semaine. Jusque-là, les patients devaient appeler le 15 en cas de symptômes, pour se voir prescrire un test RT-PCR. Pour les résidents de l’Île-de-Cayenne, le prélèvement était effectué à l’Unité des maladies infectieuses (CH de Cayenne) ou à domicile par un professionnel de santé.

L’évolution épidémiologique constatée fin mai signale un début de circulation communautaire sur l’Ile de Cayenne et à Kourou, et probablement bientôt sur l’ensemble du littoral. Il est donc nécessaire d’élargir dès aujourd’hui la filière de prescription, de prélèvements et d’analyses biomédicales. Plusieurs laboratoires de l’Ile-de-Cayenne sont désormais prêts à ouvrir des drives, les médecins de ville vont pouvoir prescrire des tests à leurs patients symptomatiques et les orienter vers la prise de rendez-vous en laboratoire.

La consultation peut être assurée par téléconsultation (via Médaviz) ; la consultation et le test Covid-19 sont intégralement remboursés par la Sécurité sociale, même pour les patients sans droits. « On voulait depuis un moment basculer vers ce système de drive, a confié Clara de Bort, directrice générale de l’ARS, à FA Guyane, samedi. Il fallait, avant, laisser le temps aux laboratoires de se préparer. L’Île-de-Cayenne aura, à partir de mardi (aujourd’hui), trois drives pour que les personnes puissent venir se faire tester au Covid-19. On se prépare ainsi à ce que le virus circule en ville. Une fois testée, c’est le médecin qui, à la vue des analyses, dira à la personne ce qu’elle doit faire. Le modèle est le même que celui appliqué dans l’Hexagone. » Retrouvez également son intervention sur le sujet au micro de RDI.

- Les patients symptomatiques vivant à Cayenne ou ses environs doivent consulter leur médecin traitant ou un médecin via Medaviz. Celui-ci prescrit un test s’il estime que le patient présente des symptômes évocateurs du COVID19. Le patient est invité à prendre immédiatement rendez-vous par téléphone auprès d’un des laboratoires de son choix :

- Les patients symptomatiques vivant à Kourou ou ses environs doivent consulter leur médecin traitant. Celui-ci prescrit un test s’il estime que le patient présente des symptômes évocateurs du COVID19. Le patient peut prendre rendez-vous dans un laboratoire à Cayenne ou être prélevé à domicile par un infirmier libéral, en appelant au 0594 32 77 62. Très prochainement, le laboratoire ouvrira lui aussi un Drive pour faciliter et augmenter ces prélèvements.
L’analyse est assurée par le laboratoire Carage.

- Les patients symptomatiques vivant à Saint-Laurent du Maroni ou ses environs doivent consulter leur médecin traitant. Celui-ci prescrit un test qui réalisé par un professionnel de santé. Le test est effectué dans un laboratoire de Kourou ou de Cayenne, en attendant que le laboratoire du CHOG soit en mesure d’assurer lui-même les analyses.

Dans tous les cas le résultat est transmis par le laboratoire au patient, au médecin prescripteur et à l’ARS. S’il est positif, le médecin prescripteur, après évaluation médicale, propose au patient soit un isolement à domicile avec suivi ou télé-suivi « Véyé mo santé », soit un isolement à l’hôtel du Fleuve, ou prescrit son hospitalisation. Il prescrit également un test aux autres membres du foyer. De leur côté, l’ARS et SPF lancent l’enquête de contact tracing et délivrent un « bon de dépistage » aux personnes hors du foyer ayant eu un contact prolongé avec la personne malade, pendant sa période de contagiosité (à partir de 48h avant le début des signes).

- Dans toutes les autres zones de Guyane, c’est le CDPS qui assure la consultation, le prélèvement et la remise du résultat du test COVID19.

- Par ailleurs, d’autres personnes doivent bénéficier d’un test de dépistage. Il s’agit des « sujets contacts » c’est-à-dire les personnes repérées par l’ARS et Santé publique France dans le cadre des démarches de « contact tracing » c’est-à-dire lors de l’interrogatoire d’un cas confirmé : quel a été votre agenda depuis que vous êtes malade et 48h avant ? Qui avez-vous croisé ? A quelle distance ? Combien de temps êtes-vous restés ensemble ? Aviez-vous un masque ? Si les épidémiologistes estiment qu’une personne a eu un contact à risque « modéré ou élevé » (un quart d’heure à moins d’un mètre sans masque, par exemple à l’occasion d’un repas), alors cette personne reçoit un « bon de dépistage ». L’ARS lui indique où et quand aller se faire prélever.

- Les patients symptomatiques se rendant spontanément aux urgences du CHC effectuent un test sur place. Si le résultat est négatif, le CHC en informe le patient par SMS ou internet. S’il est positif, l’annonce en est faite par la Garde médicale de Cayenne au patient, à son médecin généraliste et à l’ARS-SPF. Le médecin traitant, après évaluation médicale, propose au patient soit un isolement à domicile avec télé-suivi, soit un isolement à l’hôtel du Fleuve, ou prescrit son hospitalisation. Il prescrira un test aux autres membres du foyer. De leur côté, l’ARS et SPF mèneront l’enquête de contact tracing et délivreront un bon pour test aux cas contact hors foyer.

-In addition, other people must benefit from a screening test. These are “contact subjects”, that is to say people identified by the ARS and Public Health France in the context of “contact tracing” procedures, that is to say during the interrogation of a confirmed case: what has been your agenda since you were sick and 48 hours before? Who did you meet? How far ? How long did you stay together? Did you have a mask? If epidemiologists consider that a person has had a “moderate or high” risk contact (a quarter of an hour less than a meter without a mask, for example during a meal), then this person receives a "screening voucher". The ARS tells him where and when to go for a sample.

- Symptomatic patients spontaneously going to the emergency room of the CHC carry out a test on the spot. If the result is negative, the CHC informs the patient by SMS or internet. If it is positive, the announcement is made by the Cayenne Medical Guard to the patient, his general practitioner and the ARS-SPF. The attending physician, after medical evaluation, offers the patient either isolation from home with remote monitoring, or isolation from the Hôtel du Fleuve, or prescribes hospitalization. He will order a test for the other members of the household. For their part, the ARS and SPF will conduct the contact tracing investigation and issue a test voucher for out-of-home contact cases.
