L'avis du Comité Territorial Scientifique du 6 juillet 2020

Ce lundi 6 juillet 2020, le Comité Territorial scientifique, installé par la Collectivité Territoriale de Guyane (CTG), s’est réuni autour de plusieurs questions majeures, relatives à la crise sanitaire liée au Covid-19. À l’issue de cette rencontre, le comité est en capacité d’émettre un certain nombre de recommandations à destination des autorités sanitaires, fondées sur l’évaluation de la situation épidémiologique sur le territoire.

Le Comité scientifique territorial :

1. Se déclare favorable à la tenue sur le territoire d’essais cliniques susceptibles de contribuer à l’avancée des recherches sur un protocole de soins contre la Covid-19, sous réserve d’une totale transparence sur les modalités de réalisation de ces essais;

2. Demande à l’ARS, face aux données contradictoires qui circulent, une totale transparence :
- sur la capacité actuelle en lits de réanimation des trois hôpitaux de Guyane ;
- sur la marge dont disposent ces établissements pour pouvoir véritablement augmenter cette capacité tant d’un point de vue humain que matériel ;
- sur la capacité de la Martinique et de la Guadeloupe à accueillir des patients guyanais supplémentaires dans le cadre d’éventuelles prochaines évacuations sanitaires ;
- sur les indicateurs relatifs aux mouvements de médecins hexagonaux en Guyane dans le cadre du recours à la réserve sanitaire, entre ceux qui arrivent et qui font l’objet d’une médiatisation systématique et ceux qui repartent sans que l’on en soit informé ;
- sur la géolocalisation des cas à l’échelle infra-communale, de manière à permettre une action mieux ciblée des acteurs de la prévention présents sur le terrain.

3. Se réjouit de l’arrivée en Guyane d’un Préfet coordinateur spécialement dédié à la crise Covid, dont il attend une clarification des missions, objectifs et moyens de l’Etat face à cette crise, ainsi qu’une transparence accrue dans la communication entre les services de l’Etat et les acteurs locaux institutionnels et associatifs.

4. Renouvelle son appel pour l’arrivée nécessaire et urgente d’un hôpital militaire de campagne disposant d’une unité de réanimation à part entière (à l’instar de celui qui avait été installé à proximité de l’hôpital de Mulhouse) en prévision du pic annoncé pour la fin du mois de juillet, que les capacités actuelles des hôpitaux de Guyane ne permettront à l’évidence pas d’absorber.

5. Se réjouit de la commande et de la diffusion par la CTG de Tests Rapides d’Orientation Diagnostique (TROD) sérologiques sur le haut-Maroni via les PMI et Centre de santé territoriaux, de façon à compléter le dispositif de tests mis en place dans toute la Guyane et 0 permettre aux populations les plus isolées d’y avoir accès.

6. Souligne la nécessité de favoriser une plus forte inclusion des médecins de ville dans le dispositif Covid, en mettant à leur disposition, en collaboration avec les Unions Régionales des Professionnels de Santé (URPS), des saturomètres destinés à permettre un repérage précoce de complications chez les patients Covid +.

A noter que :
Le Comité territorial scientifique s’est entretenu au cours de cette séance avec le guyanais Olivier Hermine, Hématologue à l’Hôpital Necker, à Paris, et investigateur d’essais cliniques envisageant la piste du tocilizumab dans le cadre des soins de cas sévères, afin de bénéficier de son expertise. C’est d’ailleurs guidé par cette même volonté que le Président Rodolphe Alexandre a souhaité inviter ce professeur à venir en Guyane à la rencontre des médecins impliqués chaque jour dans la lutte contre le Covid-19, invitation à laquelle ce dernier a répondu favorablement, avec une arrivée prévue le 9 juillet prochain.


This Monday, July 6, 2020, the Scientific Territorial Committee, set up by the Territorial Collectivity of French Guiana (CTG), met around several major questions, relating to the health crisis linked to Covid-19. At the end of this meeting, the committee is in a position to issue a number of recommendations to the health authorities, based on the assessment of the epidemiological situation in the territory.

The Territorial Scientific Committee:

1. Is in favor of holding clinical trials in the territory likely to contribute to the advancement of research on a treatment protocol against Covid-19, subject to total transparency on the methods of carrying out these trials ;

2. Asks the ARS, in the face of contradictory data circulating, for total transparency:
- on the current capacity in intensive care beds at the three hospitals in French Guiana;
- on the margin available to these establishments to be able to genuinely increase this capacity, from both a human and a material point of view;
- on the capacity of Martinique and Guadeloupe to receive additional Guyanese patients in the context of possible future medical evacuations;
- on the indicators relating to the movements of French doctors in French Guiana within the framework of recourse to the health reserve, between those who arrive and who are subject to systematic media coverage and those who leave without being informed;
- on the geolocation of cases on an infra-municipal scale, so as to allow better targeted action by the prevention actors present on the ground.

3. Welcomes the arrival in French Guiana of a coordinating Prefect specially dedicated to the Covid crisis, from whom he awaits a clarification of the missions, objectives and means of the State in response to this crisis, as well as increased transparency in communication between state services and local institutional and associative actors.

4. Renews its appeal for the necessary and urgent arrival of a military field hospital with a full-fledged resuscitation unit (like that which had been set up near the Mulhouse hospital) by forecast of the peak announced for the end of July, which the current capacities of hospitals in French Guiana will obviously not allow to absorb.

5. Welcomes the order and distribution by the CTG of Rapid Diagnostic Orientation Tests (TROD) in Haut-Maroni via the PMI and Territorial Health Center, so as to complete the testing system put in place throughout French Guiana and 0 allow the most isolated populations to have access to it.

6. Stresses the need to encourage greater inclusion of city doctors in the Covid system, by providing them, in collaboration with the Regional Unions of Health Professionals (URPS), saturometers intended to allow early detection of complications in Covid + patients.

To note that :
The Territorial Scientific Committee met during this session with the Guianese Olivier Hermine, Hematologist at Necker Hospital, in Paris, and investigator of clinical trials considering the tocilizumab track in the context of severe case care, in order to to benefit from his expertise. It is also guided by this same desire that President Rodolphe Alexandre wished to invite this professor to come to French Guiana to meet the doctors involved every day in the fight against Covid-19, an invitation to which the latter responded favorably, with an arrival scheduled for July 9th.
