Les ambulanciers s’apprêtent à lancer une garde H24 sur tout le littoral

L’épidémie a obligé les transporteurs sanitaires à revoir leur organisation, entre port d’équipements de protection individuelle et désinfection de leurs véhicules après avoir pris en charge un patient contagieux. Au cours des grandes vacances, ils vont se structurer afin de pouvoir répondre à toutes les sollicitations du Samu 973.

Ces quinze derniers mois ont provoqué bien des chambardements, chez les ambulanciers de Guyane. Renforcement des mesures de protection lors du transport de patients atteints de Covid-19, désinfection des véhicules, garde ambulancière Covid d’avril à septembre, participation aux évacuations stratégiques… Les prochains mois vont connaître leur lot de changement également. Dès les grandes vacances, la permanence ambulancière sera renforcée. Dans l’Île-de-Cayenne, deux ambulances privées, au minimum, pourront être sollicitées par le Centre 15 pour le transport de patients vers l’hôpital. Le reste du littoral sera aussi couvert, tant lors de la garde de jour (7 heures – 19 heures) que celle de nuit (19 heures – 7 heures).

Vingt et une sociétés travaillent sur toute la Guyane. Gérard Francourt, est le président de l’Association transport et soins d’urgence (Atsu Guyane) depuis trente-deux ans. Lionel Louisor, lui, est le doyen, avec une année de plus sur les routes. Mardi matin, avec ses ambulanciers et deux véhicules, il était à l’entrée des urgences. Leur mission du jour : transporter deux patients de réanimation Covid jusqu’à la base aérienne de l’armée de l’air, avant leur évacuation stratégique (Stratévac) vers la Martinique.

Comme pour tout patient infecté au Covid-19, les ambulanciers ont enfilé une surblouse, des gants, en plus du masque. « Les EPI (équipements de protection individuels) nous sont fournis par l’ARS, explique Gérard Francourt. Je les récupère auprès de l’Adap Pro Services, puis je les distribue entre les différentes sociétés d’ambulances. »

Les ambulanciers de chez Louisor sont rompus à l’habillage. L’an dernier, ils avaient déjà effectué treize évacuations stratégiques. Et des patients atteints de Covid-19, c’est tous les jours qu’ils en transportent. Après le transport du patient contagieux, quel que soit son état de santé, il faut désinfecter le véhicule. « Dès lors que l’on a transporté un patient Covid, le véhicule ne peut plus être utilisé. Il faut compter une heure pour chaque désinfection », précise Lionel Louisor.

« Les ambulanciers jouent un rôle essentiel dans la prise en charge des patients, les rassurant, leur parlant tout au long du trajet, les accompagnant dans leurs démarches », souligne Corinne Clamecy, chargée du transport sanitaire à l’ARS Guyane. Gérard Francourt constate que même après plus d’un an d’épidémie, les réactions des patients qu’il transporte reste les mêmes, souvent teintées d’appréhension.

L’année 2021 sera l’année d’une grande rénovation de l’organisation des transports sanitaires par l’implication encore plus importante des ambulanciers dans l’aide médicale urgente. Une garde H24 sera déployée sur tout le territoire afin de répondre à toutes les sollicitations du Samu 973.

En octobre 2020, l’ARS a mis en place un « comité de suivi de la permanence ambulancière » mensuel, réunissant les sapeurs-pompiers, le Samu, l’Assurance maladie et l’hôpital de Cayenne. Cette commission permet de discuter sur tous les sujets concernant les transports sanitaires.

Dès les grandes vacances, un logiciel et un poste de coordonnateur ambulancier seront installés au sein du Samu afin de rendre plus efficient l’envoi d’une ambulance auprès du patient et faire diminuer les carences ambulancières.

Depuis novembre 2020, un groupe de travail a été mis en place avec un cabinet de Paris afin de mettre en place une plateforme territoriale d’appui pour les transports complexes dont le but est :

Le renforcement de l’offre de soins sur la Guyane (connaissance, développement, confiance) avec une meilleure gestion des transports complexes : une des priorités du projet régional de santé ;
L’optimisation du temps médico-soignant en sécurisant et allégeant les processus logistiques des transports.

Bien d’autres projets seront étudier au cours des mois à venir.

These last fifteen months have caused many upheavals among the ambulance staff in French Guiana. Strengthening of protective measures during the transport of patients with Covid-19, disinfection of vehicles, Covid ambulance guard from April to September, participation in strategic evacuations ... The coming months will also see their share of changes. From the summer holidays, the ambulance service will be reinforced. In Cayenne, at least two private ambulances may be requested by Center 15 to transport patients to the hospital. The rest of the coast will also be covered, both during the day (7 am - 7 pm) and night (7 pm - 7 am) guard.

Twenty-one companies work throughout French Guiana. Gérard Francourt, has been the president of the Association for transport and emergency care (Atsu Guyane) for thirty-two years. Lionel Louisor is the dean, with one more year on the road. Tuesday morning, with his paramedics and two vehicles, he was at the emergency entrance. Their mission for the day: to transport two Covid resuscitation patients to the air force base, before their strategic evacuation (Stratévac) to Martinique.

As with any patient infected with Covid-19, the paramedics put on a gown, gloves, in addition to the mask. "PPE (personal protective equipment) is supplied to us by the ARS," explains Gérard Francourt. I collect them from Adap Pro Services and then distribute them among the various ambulance companies. "

Louisor's paramedics are well versed in dressing. Last year, they had already carried out thirteen strategic evacuations. And patients with Covid-19, they carry it every day. After transporting the contagious patient, regardless of their state of health, the vehicle must be disinfected. “Once a Covid patient has been transported, the vehicle can no longer be used. It takes an hour for each disinfection, ” explains Lionel Louisor.

"Paramedics play an essential role in the care of patients, reassuring them, talking to them throughout the journey, accompanying them in their efforts", underlines Corinne Clamecy, in charge of medical transport at ARS French Guiana. Gérard Francourt notes that even after more than a year of the epidemic, the reactions of the patients he carries remain the same, often tinged with apprehension.

The year 2021 will be the year of a major overhaul in the organization of medical transport through the even greater involvement of ambulance staff in urgent medical aid. A 24-hour guard will be deployed throughout the territory in order to respond to all requests from the Samu 973.

In October 2020, the ARS set up a monthly "ambulance duty monitoring committee", bringing together firefighters, Samu, Health Insurance and Cayenne Hospital. This committee allows discussions on all subjects concerning medical transport.

From the summer holidays, software and an ambulance coordinator station will be installed within the Samu in order to make sending an ambulance to the patient more efficient and reduce ambulance deficiencies.

Since November 2020, a working group has been set up with a Paris cabinet in order to set up a territorial support platform for complex transport, the aim of which is:

Strengthening the healthcare offer in French Guiana (knowledge, development, confidence) with better management of complex transport: one of the priorities of the regional health project;
Optimization of medico-nursing time by securing and reducing the logistical processes of transport.

Many more projects will be explored in the coming months.
