COVID : un essai sauvage provoque un scandale au Brésil

Notre voisin le Brésil est actuellement secoué par un scandale sanitaire, autour d’un possible essai thérapeutique sauvage, aux résultats falsifiés. Un groupe d’hôpitaux aurait utilisé un cocktail d’hydroxychloroquine, d’azithromycine et d’ivermectine pour « traiter » des centaines de patients atteints de Covid-19, sans leur signaler qu’ils participaient à un essai thérapeutique. Au moins neuf seraient morts, selon des médecins de ces hôpitaux, qui ont dénoncé l’affaire. Toute mention du Covid-19 aurait été supprimé de leur dossier médical, pour « améliorer » les résultats de ce traitement dans leurs conclusions.

Hydroxychloroquine, azithromycine et ivermectine. Ces prétendus médicaments miracle contre le Covid-19 sont régulièrement vantés sur les réseaux sociaux et dans certaines manifestations, notamment en Guyane. Pourtant, leur efficacité à prévenir les infections, les formes graves et les décès liés au Covid-19 n’a jamais été démontrée. Ni pour l’ivermectine (lire cette revue Cochrane), ni pour l’azithromycine (lire l’article de l’essai Recovery dans le British Journal of Medicine), ni pour l’hydroxychloroquine (lire les recommandations de l’Organisation mondiale de la santé sur les médicaments contre le Covid-19). Prescrire ou même recommander ces traitements non éprouvés enfreint les codes de déontologie des professionnels de santé et le code de la santé publique. C’est aussi dangereux car créant un faux sentiment de sécurité pour les patients.

Au Brésil, ce « cocktail » est au cœur d’un scandale qui touche un groupe d’hôpitaux et le président Jaïr Bolsonaro. Ce « cocktail » constituait le « Kit Covid », ainsi que l’appelait le groupe d’hôpitaux brésilien Prevent Senior. Il aurait été utilisé dans le cadre d’un essai non autorisé. Selon les éléments diffusés par la presse brésilienne, des patients auraient reçu ce kit puis, en majorité, auraient été renvoyés chez eux et suivis par téléconsultation. Au moins neuf seraient morts après avoir reçu ce traitement. Toute référence au Covid-19 aurait été supprimée de leur dossier, afin d’améliorer les résultats du traitement dans l’essai promu par ces hôpitaux, a relaté l’avocate Bruna Morato, lors d’une enquête du Sénat. Elle défend douze médecins du groupe hospitalier, qui dénoncent cet essai et accusent leur employeur d’avoir fait pression sur eux pour qu’ils prescrivent ce « traitement ». La direction du groupe hospitalier rejette ces accusations.

« Des patients âgés très vulnérables ont été informés qu’il existait un bon traitement, mais ils ne savaient pas qu’ils étaient utilisés comme cobayes », a déclaré l’avocate des dénonciateurs. Elle a ajouté que les médecins avaient reçu l’ordre de ne pas expliquer le traitement aux patients ou à leurs proches. « L’objectif était de montrer qu’il existait un traitement efficace contre le Covid-19 », a-t-elle déclaré. Avant d’ajouter que l’hôpital avait passé un accord pour aider le gouvernement Bolsonaro, qui présentait ces médicaments non éprouvés comme un traitement efficace contre le virus, qui protégerait les Brésiliens de la contagion s’ils reprenaient le travail.

En août, les médecins dénonciateurs ont remis aux enquêteurs un dossier de 10 000 pages contenant une série d'allégations incendiaires à l’encontre de l’entreprise basée à São Paulo. Le dossier contenait des allégations selon lesquelles des patients âgés avaient été utilisés comme « cobayes humains » pour tester des « remèdes » Covid non éprouvés, sans avoir donné leur plein consentement.

Pour l’avocate Bruna Morato, « cela coûte bien moins cher à un groupement hospitalier de donner ces médicaments plutôt que d’hospitaliser les patients ». L’un d’eux a obtenu la condamnation de Prevent Senior à 2 millions de reais (320 000 euros) de dommages et intérêts. Malgré ses symptômes graves, il n’avait pas été placé en soins intensifs et avait reçu de l’hydroxychloroquine et de l’ivermectine. « Ils m’ont donné un kit Covid mais ils ne m’ont pas donné d’oxygène, a-t-il témoigné dans la presse brésilienne. Comment pouvais-je aller mieux si je ne pouvais même plus respirer ? » Il a finalement été hospitalisé pendant 35 jours. Aujourd’hui, il souffre de la paralysie d’un bras et d’une insuffisance rénale. Un médecin envoyé à son chevet par sa famille a fini par constater les manquements de Prevent Senior.

Ce scandale sanitaire rappelle l’importance de l’encadrement des essais thérapeutiques, et la nécessité d’offrir aux patients des traitements éprouvés. Si des traitements sont en phase d’étude, le malade doit être pleinement informé, consentent et parfaitement suivi. Les résultats doivent être honnêtes et transparents. Il est interdit et immoral de s’improviser chercheur et de jouer avec la santé des patients.

This health scandal is a reminder of the importance of supervising therapeutic trials, and the need to offer patients proven treatments. If treatments are in the study phase, the patient must be fully informed, consented and fully monitored. The results must be honest and transparent. It is forbidden and immoral to improvise as a researcher and to play with the health of patients.
