Avec sa nouvelle certification, la HAS atteste des progrès réalisés au CHC

La semaine dernière, le CHC a été certifié « avec recommandations d’amélioration », pour une durée de quatre ans, par la Haute Autorité de santé. Il obtient la note B, sur une échelle allant de A (certification) à E (non-certification). Jamais l’établissement cayennais n’avait reçu une telle appréciation. La HAS souligne notamment l’amélioration du respect des droits des patients, en particuliers en psychiatrie. Elle encourage la poursuite de la réhabilitation du secteur de psychiatrie et celle à venir du secteur de médecine.

Une certification avec « obligations » d’amélioration en 2018 ; une certification avec « recommandations » d’amélioration quatre ans plus tard. En changeant un mot, la Haute Autorité de santé (HAS), chargée d’évaluer la qualité et la sécurité des soins dispensés et l'ensemble des prestations délivrées par les établissements de santé, atteste des progrès importants réalisés au centre hospitalier de Cayenne (CHC) au cours des dernières années. Jeudi dernier, elle a publié sa décision de certifier l’établissement « avec recommandations d’amélioration pour une durée de quatre ans ». Le CHC est désormais classé B, sur une échelle allant d’A (certification) à E (non-certification). Jamais le CHC n’avait obtenu une aussi bonne note.

Pour mesurer le chemin parcouru, il faut se souvenir que la HAS avait décidé de surseoir à sa décision de certification de l’hôpital de Cayenne en 2013, qu’elle avait émis des réserves en 2014 et demander à recevoir des plans d’actions sous trois mois, qu’elle avait imposé des obligations en 2018 et attribué la note C, avant sa décision de cette année. Il faut aussi se pencher sur le rapport de 2018.

« Le travail a porté ses fruits »

Le service de psychiatrie – devenu pôle santé mentale – « était une thématique majeure sur laquelle la HAS avait pointé le doigt », se souviennent Aurore Nemer, directrice adjointe chargée notamment de la qualité et de la gestion des risques au CHC, et Dominique Toul, son adjointe. La Haute Autorité de santé avait souligné plusieurs difficultés au regard du respect des droits des usagers, comme nous le rappelions lors de l’ouverture de l’unité Wapa rénovée (Lire la Lettre pro du 17 juin). « Cela faisait partie de nos obligations d’amélioration. Nous avons travaillé très fort l’année dernière. Il faut rendre hommage à la cheffe de pôle le Dr Caroline Janvier, au cadre Jean-Michel Hilaire, et à tous les soignants, les ASH, les aides-soignants, les infirmiers, les médecins et tout le personnel. Le service fermé de soins sous contrainte a été totalement rénové avec des chambres d’isolement moins nombreuses et surtout beaucoup plus adaptées, mais aussi un espace d’apaisement, un espace pour les familles et une nette amélioration de l’environnement des usagers et des conditions de travail des soignants. » Dominique Toul reconnaît avoir « insisté auprès des équipes au sujet des méthodes de qualité, les patients traceurs… mais ça a porté ses fruits. Nous avons démarré un travail de fond et nous tenons un comité de pilotage qualité tous les mois dans le service. »

D’autres chantiers ont été menés en matière de droits des patients : refonte du livret d’accueil des patients hospitalisés, distribution aux professionnels d’un livret sur l’identito-vigilance, édition de flyers et bientôt mise à disposition de QR Codes sur l’accès au dossier médical, les directives anticipées, la personne de confiance… Le CHC a également mis en place, l’an dernier, son comité de bientraitance. Composé de personnels médicaux et non médicaux, ainsi que de représentants des usagers, il permet de mener des actions de formation, de mener un audit sur la bientraitance et doit se doubler de la mise en service d’un outil de déclaration de la maltraitance distinct de l’outil de déclaration des événements indésirables.

Le chantier de réhabilitation de l’hôpital à poursuivre

Dans le courrier accompagnant sa décision, la HAS « souligne les très importants travaux qui ont été menés par le centre hospitalier de Cayenne pour parvenir à un hébergement des patients, notamment relevant des secteurs de la santé mentale, respectueux de leurs droits. Elle encourage la poursuite du schéma directeur immobilier arrêté. » Pour Aurore Nemer, la HAS vient reconnaître que la qualité et la sécurité de la prise en charge des patients « tout en maintenant la pression sur la réhabilitation de l’hôpital. Les travaux sont prévus. » Elle souhaite que l’établissement poursuive la réhabilitation du secteur de psychiatrie et celle à venir du secteur de médecine.

Avec cette reconnaissance des efforts réalisés, Aurore Nemer et Dominique Toul espèrent « pouvoir mobiliser tous les professionnels du CHC, de façon à ce qu’il y ait une synergie pour la prochaine certification. On commence à travailler sur la certification V2020. » Dominique Toul constate que « c’est une certification vraiment centrée sur le patient. Celui-ci veut dire qu’on le soigne mais aussi qu’on lui parle, qu’on travaille avec lui. La certification n’est pas un but en soi : nos patients, c’est tous les jours qu’ils font la certification. »

The Haute Autorité de santé had highlighted several difficulties with regard to respect for the rights of users, as we recalled during the opening of the renovated Wapa unit (Read the Professional letter of June 17). “It was part of our improvement obligations. We worked very hard last year. We must pay tribute to the head of the center Dr Caroline Janvier, to the executive Jean-Michel Hilaire, and to all the caregivers, the ASH, the caregivers, the nurses, the doctors and all the staff. The closed care service under duress has been completely renovated with fewer and above all much more suitable isolation rooms, but also a calming space, a space for families and a marked improvement in the environment for users and working conditions of caregivers. Dominique Toul acknowledges having “insisted with the teams on the subject of quality methods, tracer patients… but it paid off. We have started substantive work and we hold a quality steering committee every month in the department. »

Other projects have been carried out in terms of patients' rights: redesign of the welcome booklet for hospitalized patients, distribution to professionals of a booklet on identity-vigilance, publication of flyers and soon provision of QR Codes on access to medical records, advance directives, the person of trust… Last year, the CHC also set up its well-treatment committee. Composed of medical and non-medical personnel, as well as user representatives, it allows training actions to be carried out, an audit to be carried out on well-treatment and must be coupled with the implementation of a separate tool for reporting mistreatment. of the adverse event reporting tool.

The hospital rehabilitation project to continue

In the letter accompanying its decision, the HAS “underlines the very important work that has been carried out by the Cayenne hospital center to achieve accommodation for patients, in particular in the mental health sectors, respectful of their rights. It encourages the continuation of the real estate master plan adopted. For Aurore Nemer, the HAS recognizes that the quality and safety of patient care “while maintaining pressure on hospital rehabilitation. The work is planned. She would like the establishment to continue the rehabilitation of the psychiatry sector and that to come of the medicine sector.

With this recognition of the efforts made, Aurore Nemer and Dominique Toul hope “to be able to mobilize all CHC professionals, so that there is synergy for the next certification. We are starting to work on V2020 certification. » Dominique Toul notes that « it is a certification really centered on the patient. This means that we treat him but also that we talk to him, that we work with him. Certification is not an end in itself: our patients are certified every day. »
