Point d’étape sur l’élaboration du projet médical du CHRU de Guyane

Le projet de Centre Hospitalier Régional Universitaire de Guyane, qui s’appuiera sur les trois établissements publics de Guyane (CHC, CHK et CHOG) et sur les hôpitaux de proximité, constitue un engagement fort de l’Etat pour améliorer l’accès aux soins des guyanaises et des guyanais, rendre le système de santé du territoire plus attractif pour les professionnels et faire de la Guyane un terrain d’universitarisation, de recherche et de formation.

Cette ambition fait l’objet d’un travail collaboratif engagé depuis janvier 2022, avec l’ensemble des acteurs de santé du territoire, sous l’égide de l’ARS et d’un comité de pilotage territorial sous la co-présidence du Préfet et du Président de la CTG, et réunissant les élus et les autres parties-prenantes Au cours des derniers mois, différents groupes de travail ont été organisés avec les communautés médicales des trois hôpitaux afin de coconstruire un projet médical d’excellence pour le futur CHRU de Guyane.

Les orientations qui découlent de ces travaux sont les suivants :

- L’universitarisation de l’offre de soins pour développer une recherche originale et correspondant aux besoins en santé de la population ; cet engagement visera en particulier la mise en place d’une unité mixte de recherche en santé
- La coordination de l’offre de soins qui se traduira par des équipes territoriales renforcées ; elles pourront être multi-site ou centralisées avec des missions sur d’autres sites (y compris dans les hôpitaux de proximité) ;
- La territorialisation des parcours de soins afin d’éviter les ruptures de prises en charge et de limiter les pertes de chance pour les patients, en facilitant le déploiement d’équipes mobiles ;
- L’attractivité territoriale par le développement des formations dans une approche partenariale avec l’ensemble des acteurs pour développer les compétences en Guyane et fidéliser les professionnels.

L’organisation de plusieurs filières et parcours de soins fait d’ores et déjà l’objet d’un consensus entre les acteurs, les sujets encore ouverts feront l’objet de propositions d’ici la fin du mois d’août pour permettre à l’ARS d’examiner la conformité de ce projet médical par un avis qu’elle publiera à la fin du mois d’octobre.

La qualité des discussions entre les parties-prenantes permet de clarifier plusieurs points d’organisation des soins :

- Le projet médical tel qu’il se dessine permettra de développer l’activité sur chacun des sites avec notamment la consolidation de la chirurgie, permise par l’universitarisation de certaines filières identifiées comme prioritaires : chirurgie digestive et chirurgie orthopédique ;
- Le développement des investissements hospitaliers avec la programmation des mesures actées dans le cadre du Ségur de la santé ; le premier robot chirurgical de Guyane qui sera installé au centre hospitalier de Kourou début septembre grâce aux fonds européens FEDER et d’autres opérations d’investissements, poursuivront la revitalisation de ce site ;
- Le maintien et la modernisation des services d’urgence de Cayenne, Kourou et St Laurent du Maroni, en amplifiant les coopérations entre les sites, en développant les transports internes pour limiter les pertes de chance, grâce à une approche d’équipe de territoire et le développement des transports sanitaires (aériens ou en pirogue) ;
- La structuration des prises en charge Femme-Mère-Enfant, pour relever le défi de la croissance démographique, en confortant les 3 maternités des établissements publics de santé et en développant l’expertise de gynécologie ainsi que l’offre de soins pédiatriques, de la phase aigüe à la prise en charge de l’aval.

Des consultations des instances hospitalières (conseil de surveillance et commission médicale d’établissement mais aussi instances du GHT) seront organisées dans les prochaines semaines. Le comité de pilotage territorial, associant toutes les parties-prenantes, se réunira quant à lui à la rentrée.

The project for the Center Hospitalier Régional Universitaire de Guyane, which will rely on the three public establishments in French Guiana (CHC, CHK and CHOG) and on the local hospitals, constitutes a strong commitment by the State to improve access to care for Guyanese men and women, make the territory's health system more attractive for professionals and make Guyana a land of university studies, research and training.

This ambition has been the subject of collaborative work undertaken since January 2022, with all the health actors in the territory, under the aegis of the ARS and a territorial steering committee under the co-chairmanship of the Prefect and the President of the CTG, and bringing together elected officials and other stakeholders In recent months, various working groups have been organized with the medical communities of the three hospitals in order to co-construct a excellent medical project for the future CHRU of French Guiana.

The orientations resulting from this work are as follows:

- The universitarization of the care offer to develop original research that meets the health needs of the population; this commitment will focus in particular on setting up a joint health research unit
- The coordination of the care offer which will result in stronger territorial teams; they can be multi-site or centralized with missions on other sites (including in local hospitals);
- The territorialization of care pathways in order to avoid disruptions in care and limit the loss of opportunity for patients, by facilitating the deployment of mobile teams;
- Territorial attractiveness through the development of training in a partnership approach with all stakeholders to develop skills in French Guiana and retain professionals.

The organization of several sectors and care pathways is already the subject of a consensus between the actors, the still open subjects will be the subject of proposals by the end of August to allow the ARS to examine the conformity of this medical project by an opinion which it will publish at the end of October.

The quality of the discussions between the stakeholders makes it possible to clarify several points of organization of care:

- The medical project as it is taking shape will make it possible to develop activity on each of the sites with, in particular, the consolidation of surgery, made possible by the universitization of certain sectors identified as priorities: digestive surgery and orthopedic surgery;
- The development of hospital investments with the programming of measures taken within the framework of Ségur de la santé; the first surgical robot in Guyana, which will be installed at the Kourou hospital center at the beginning of September thanks to European ERDF funds and other investment operations, will continue the revitalization of this site;
- Maintaining and modernizing emergency services in Cayenne, Kourou and St Laurent du Maroni, by increasing cooperation between sites, developing internal transport to limit loss of chance, thanks to a territorial team approach and the development of medical transport (air or canoe);
- The structuring of Women-Mother-Child care, to meet the challenge of population growth, by strengthening the 3 maternity wards of public health establishments and developing gynecological expertise as well as the offer of pediatric care, from the acute phase to downstream care.

Consultations with hospital authorities (supervisory board and establishment medical commission as well as GHT authorities) will be organized in the coming weeks. The territorial steering committee, bringing together all the stakeholders, will meet at the start of the school year.
